Intussusception in a 1 year old MN DSH cat

This 1 year old MN DSH cat has been chronically underweight despite good appetite. No reported vomiting or diarrhea. On Cobalamin oral supplementation.

DX

Diffuse intestinal thickening with intussusception.

Image Interpretation

The kidneys were normal in size and contour; however, subtle medullary rim sign was noted. Medullary rim sign in the kidneys is an idiopathic finding; however, this can be associated with FIP.

Intussusception was noted in this patient with pericapsular inflammatory pattern. This appeared to be at the ileocecal junction and is relatively small. . Diffusely thickened intestine was noted with mesenteric lymphadenopathy. Soft stool was noted in the colon. Multiple mesenteric lymph nodes were enlarged with a cluster of which that measured approximately 2.0 x 3.0 cm.  A slight amount of free fluid was noted between the liver lobes.

Outcome

Variable intestinal thickening. Trace free fluid.
Surgical exploratory is recommended in this patient with mesenteric lymph nodes
biopsies. Reduction of the intussusception at the ileocecal region and renal +/- hepatic
biopsies. Chronic inflammatory bowel disease, emerging lymphoma and FIP are the
primary differentials. Guarded prognosis depending upon histopathology results.
