Intestinal adenocarcinoma and mural abscess in a 10 year old FS Siamese cat

This 10-year-old spayed female Siamese cat presented for anorexia and mild weight loss. Mild dehydration was noted, with a palpable upper intestinal thickening on physical exam. CBC demonstrated a mild leukocytosis with a left shift. Blood chemistry was essentially normal, with only mildly elevated lipase and mildly elevated CPK.

Sonographic Differential Diagnosis

Intestinal mass-adenocarcinoma, lymphoma, granulomatous lesion with abscessation, leiomyosarcoma, other neoplasia.

Image Interpretation

A longitudinal image of a segment of small bowel demonstrates marked, diffuse wall thickening with complete loss of layering. Echogenic ill-defined omentum is evident consistent with localized inflammation. A second image of adjacent bowel from the same patient reveals a well-encapsulated, round, centrally cavitated, moderately thick-walled mass arising from the thickened bowel wall noted on the first image. The video demonstrates discrete transition from relatively normal intestinal wall into the abscessing mass.

Abscessing intestinal adenocarcinoma

Outcome

The patient was euthanized after 1 month of relatively poor response post surgery. Chemotherapy was never initiated.

Clinical Differential Diagnosis

Neoplasia, inflammatory bowel disease with lymphadenopathy, foreign body, pancreatitis, FIP, intussusception.

Sampling

Surgical resection revealed intestinal adenocarcinoma.
