DX

Infiltrative small intestinal mass with barbell type appearance extending to the proximal cloaca

Image Interpretation

A 2 x 3 cm infiltrative intestinal mass was noted in the caudal abdomen. This appears to be tapering into the cloaca. This appears resectable and is an annular mass with entrapped chyme prior to the lesion. A separate infiltrative pattern was noted in a barbell type appearance with a second portion of the mass measuring another 2.0 cm. Therefore, these are two separate masses with a barbell type appearance of infiltrative tissue extending for approximately 5-6 cm in its entirety. The cloaca appears to be intact. Surrounding tissues and other portions of intestine were unremarkable. The upper gastrointestinal tract did not appear to be affected. Portions of the mass were surrounded by reactive mesentery creating a peritonitis. There was no free fluid noted at this time.

Outcome

Ultrasound-guided FNA is strongly recommended for further definition
of this type of mass followed by either surgical intervention with attempt at resection
and adjunctive chemotherapy. Intestinal lymphoma, carcinoma, and granulomatous
disease are the primary differentials.
