Ileocecal mass in a 14 year old MN DSH

A 14-year-old MN DSH cat was presented for chronic weightloss despite moderate thyroid regulation, with a history of known hyperthyroidism on Methimazole.

Sonographic Differential Diagnosis

cecal carcinoma, lymphoma, granulomatous disease or less likely dry form FIP

Image Interpretation

An ileocecal mass was noted and was mixed, hypoechoic and irregular measuring 2.5 x 1.9 cm. Variable small intestinal thickening was noted elsewhere with regional mesenteric lymphadenopathy. The ileocecal mass appeared to be deriving from the proximal cecum. Regional lymph nodes were enlarged and measured 1.0 x 0.5 cm with reactive mesentery. The mass derived from the cecum and is potentially resectable; however, given the lymph node involvement regional spread is probable.

DX

Ileocecal mass with variable small intestinal thickening elsewhere and mesenteric lymphadenopathy

Outcome

Ultrasound-guided FNA of the cecal mass and lymph nodes would be recommended if possible. Otherwise, surgical approach to ileocecocolic resection, lymph node removal and biopsies would be indicated. Differentials include cecal carcinoma, lymphoma, granulomatous disease or less likely dry form FIP. Treatment for chronic cystitis would also be warranted over a 4-6 week period. Ursodiol therapy would be recommended over the next 4-6 weeks.

Sampling

Ultrasound-guided FNA of the cecal mass and lymph nodes would be recommended if possible.
