A 13-year-old NM Bichon Frise dog was presented with a history of hypercalcemia, elevated parathyroid hormone, and liver enzyme activity.

DX

Parathyroid adenoma based on elevated PTH and appropriate parathyroid nodule

Sonographic Differential Diagnosis

Hypoechoic nodule in the region of the parathyroid. Likely parathyroid adenoma. Nodular hyperplasia possible.

Image Interpretation

The right and left lobes of the thyroid gland were imaged. The left measured 1.94 x 0.3 cm with uniform parenchyma and contour. The right was uniform with minor remodeling measuring 1.2 x0.6 cm. However, a hypoechoic nodule was noted adjacent to the right thyroid lobe measuring 0.84 x 0.3 cm, which is consistent with parathyroid adenoma. If the sonographer is comfortable with the procedure a 25 gauge FNA of this nodule would be recommended to ensure that no neoplasia is present. Alternatively, removal of the parathyroid surgically would be recommended, especially if calcium levels continue to increase.

Outcome

The patient responded to medical therapy and the was lost for further follow-up
