Histiocytic sarcoma diagnosed on FNA in an 8 year old MI Rottweiler

The patient is an 8 year old MI Rottweiler dog. CBC showed severe, non regenerative anemia, while the blood chemistry showed low albumin at 1.9 and a mildly elevated ALP.

DX

Splenic histiocytic sarcoma

Sonographic Differential Diagnosis

Splenic mass and nodules suggestive of neoplasia. The liver is likely also involved in the neoplastic process.

Image Interpretation

The spleen presented aggressive nodules with parenchymal changes. There was a splenic nodule measuring 1.38 x 1.4 cm in the mid-splenic body. A separate splenic mass measured 3.46 x 2.66 cm. The liver was swollen and mildly irregular in contour in the left medial liver lobe. The remainder of the liver and the gallbladder were unremarkable. Rapid view of the right heart revealed no evident pathology.

Outcome

No further outcome at this time.

Comments

Due to the appearance of the liver, FNA of the liver should have been considered at the same time as FNA of the spleen.

Clinical Differential Diagnosis

Nonregenerative anemia – neoplasia, GI disease (ulcers causing chronic blood loss), bone marrow pathology. Hypoalbuminemia – PLE, PLN, liver failure.

Sampling

Ultrasound-guided FNA of the spleen revealed histiocytic sarcoma.
