A 12-year-old NM Jack Russell Terrier was presented for evaluation of hematuria with one episode of stranguria. Urinalysis showed specific gravity of 1.032, 3+ blood, and 4+ protein. CBC and serum biochemistry was within reference range.

Sonographic Differential Diagnosis

Persistent urinary debris.
Cystic right renal mass. Hemangiosarcoma is the primary differentials. There was no overt evidence of metastatic disease.
Mild to moderate degenerative left renal changes.

Image Interpretation

The urinary bladder presented persistent debris. There was no evidence of calculi or distal urinary masses. The pelvic urethra was imaged 3.0 cm beyond the cystourethral junction.

The residual prostate was unremarkable.

The right kidney in this patient presented a cystic mass that measured 8.07 cm. This is potentially renal hemangiosarcoma or abscessed kidney. There was no visible, functional parenchyma. Power Doppler assessment of the right kidney was positive, which is indicative of neoplastic tissue. However, this appears to be contained in the right renal capsule. The left kidney revealed minor, degenerative changes with cortical remodeling and minor infarcts. The left kidney measured 4.0 cm. There was no evidence of neoplasia. The left kidney revealed microcystic changes.

Outcome

Three view chest radiographs followed by right nephrectomy with appropriate culture and biopsy are recommended.

Prostate – neoplasia, prostatitis
Kidneys – renoliths, neoplasia, pyelonephritis
