Gastritis (likely a granulomatous lesion) in an 11 year old FS Persian cat

An 11-year-old spayed female Persian cat was presented for vomiting. Owner reports patient had been vomiting a couple times a week for approximately one month. Physical exam was unremarkable.

DX

Gastritis with likely granuloma

Sonographic Differential Diagnosis

Gastritis with granulomatous echogenic lesion. Potential for carcinoma or FIP.

Image Interpretation

An echogenic wall thickening was noted in the pyloric outflow. The echogenic changes would suggest a chronic process but underlying carcinoma or other neoplasia is possible. No loss of mural detail suggests an inflammatory pathology.

Outcome

Biopsies of the stomach were recommended, along with azithromycin therapy, Vitamin B complex injections, and steroids. Assessment for toxoplasmosis and Bartonella were also advised. Patient was discharged with azithromycin, steroids, and Laxatone. Patient presented for recheck ultrasound and owner reported patient had been experiencing diarrhea. Ultrasound did not reveal any significant changes. A diet change to I/D was recommended and Vitamin B complex treatment was started. At last communication owner reported patient still experiencing diarrhea.

Clinical Differential Diagnosis

Trichobezoar, inflammatory bowel disease, gastritis (infectious), neoplasia (gastric or intestinal), metabolic disease (hyperthyroidism), chronic renal failure, cholangiohepatitis, pancreatitis
