A 12-year-old spayed female DSH cat was presented for evaluation of decreased appetite. Abnormalities on physical examination included weight loss and a possible abdominal mass on palpation. CBC showed neutrophilia and lymphocytopenia.

DX

Gastric lymphoma

Sonographic Differential Diagnosis

Gastric mass. This is likely lymphoma/round cell neoplasia, with a minor potential for carcinoma or granulomatous disease.

Image Interpretation

A dramatically hypoechoic, mural, concentric mass is present encompassing most of the gastric wall and invading the pyloric outflow. Complete loss of mural detail is present.

Outcome

The owners elected chemotherapy and patient was started on the CHOP protocol.

Clinical Differential Diagnosis

Chronic renal disease, liver disease, inflammatory bowel disease, intra-abdominal neoplasia (liver, spleen, GI tract), intra-abdominal granulomatous inflammation, hyperthyroidism.

Sampling

US-guided FNA (22 gauge needle) of the stomach/pylorus mass was conducted. The cytology was consistent with gastric lymphoma.
