Gastric Foreign Body in a 10 year old Terrier mix dog

This 10-year-old Terrier cross breed dog presented for persistent vomiting, hematemesis, and hypothermia that was not responsive to medical therapy. Blood analysis revealed a moderately elevated SAP, markedly increased BUN with mildly increased phosphorous, and moderately elevated lipase. Urine was concentrated with non reactive sediment.

Gastric foreign body

Sonographic Differential Diagnosis

Gastric foreign body with mild gastric stasis primary; a large polyp or pedunculated neoplasm could also be considered.

Image Interpretation

A large, irregularly shaped structure is occupying and partially conforming to a majority of the gastric body and pyloric lumen. The structure is isoechoic to the submucosal layer. On most images, the mucosal layer and in some regions, the mucosal gas interface layer are discernible and normally linear consistent with lack of communication with the intraluminal structure. In the regions where the mucosal layer is not visualized, a faint, echogenic linear or curvilinear peripheral border is suggested. The unaffected gastric lumen is mildly distended with fluid.

Outcome

The patient responded well to gastrotomy and 3 days of hospital care. Blood values normalized with a residual mildly elevate SAP.

Clinical Differential Diagnosis

GI pathology – GI neoplasia, gastroenteritis/ulcer, pancreatitis, infectious, toxicity, neoplasia.

Sampling

Surgical exploratory revealed a partially eaten potato and gastric petechiae.
