The patient is a canine mix, Intact female, 5 years. The patient has a history of a cough and lethargy. A grade V/VI left sided heart murmur was noted.

DX

Flail mitral valve. Left and right sided congestive heart failure. Pulmonary hypertension. Pericardial effusion. Suspect left atrial tear.

Sonographic Differential Diagnosis

Stage C1 valvular disease. Flail mitral valve and insufficiency. Left and right sided congestive heart failure. Pulmonic insufficiency consistent with pulmonary hypertension.

Image Interpretation

The echocardiogram for this patient presented severe left atrial enlargement. The cranial and caudal mitral valve leaflets demonstrated mildly vegetative contour. Flail mitral valve was noted and severe insufficiency. Ventricular function appeared hypercontractile yet overload was present. The right atrium was enlarged owing to tricuspid insufficiency. No neoplastic evidence was visualized here or into its extension of the right auricle.

Outcome

The patient initially responded initially to triple therapy of ace inhibitor, Lasix and Pimobendan. Sudden death at home occurred 10 days after the ultrasound.

Clinical Differential Diagnosis

Congestive heart failure, Bronchial disease, pneumonitis, thromboembolic disease.
