A 17-year-old FS DSH cat was presented for Inappetence, thickened intestines, kidney failure and hypertension. Abnormalities on CBC and serum biochemistry were increased BUN 44 and Creatinine 2.6. Potassium 3.6 slightly subnormal.

DX

Enlarged left adrenal gland, Conn's disease likely

Image Interpretation

The left adrenal gland in this patient was enlarged, hypoechoic and rounded. The left adrenal gland measured 1.63 x 1.2 cm. 

Outcome

Testing aldosterone levels recommended to assess for Conn’s disease. Hypertension is likely related to the left adrenal tumor +/- emerging renal failure. Left adrenalectomy, renal biopsy and gastrointestinal biopsies would be warranted given the diffuse intestinal thickening to rule out underlying occult lymphoma versus more likely inflammatory bowel with hypertrophied muscularis.
