Case Study

Ectopic ureter with separate pyoureter and pyelonephritis in a 3 month old F Golden Doodle

This 3-month-old intact female Golden Doodle was presented for evaluation of UTI and pollakiuria that had improved with antibiotic therapy but relapsed once the course was completed

DX

Ectopic ureter with separate pyoureter and pyelonephritis in the right kidney.

Image Interpretation

The urinary bladder, trigoneand pelvic urethra presented normal wall thicknesses with anechoic urine and normal tone. No uroliths or sediment were visualized. No evidence of inflammatory or neoplastic changes were noted.

The right kidney in this patient presented echogenic debris and dilated pelvis as well as hydroureter with echogenic debris. The right ureter was dilated at 1.0 cm and thickened to the level of the bladder trigone. The right ureter appears to enter ectopically into the dorsal, apical bladder. Hydronephrosis in the right kidney measured 1.3 cm.  The left kidney was uniform and measured 4.3 cm in length with trace pyelectasia.

Outcome

Minor pelvic dilation in the left kidney. Likely embedded infection. I recommend surgical approach in this patient with drainage of the renal pelvis and ureter as well as proper redirection of the ureter. Long term antibiotics after surgical redirection of the right ureter would be indicated. Regional inflammation was noted around the right kidney and right ureter.

Clinical Differential Diagnosis

Ectopic ureter, bladder urachus, uroliths, pyelonephritis
