Duodenal foreign body in a 4 year old MN Labrador Retriever *Sep 2010 COM*

A 4-year-old MN Labrador retriever was presented for vomiting food and bile over the prior 5 days. The owner reported no potential for foreign body consumption. Physical exam revealed mild dehydration, depression, excessive panting, mild tender cranial abdomen, and slightly slow capillary refill time (2 seconds.) Blood analysis was normal except for minor anemia and slight albumin elevation. Radiographs were unremarkable. The patient responded slightly to IV fluid support and cerenia, but had minimal appetite.

Duodenal foreign body

Sonographic Differential Diagnosis

Foreign body obstruction of the descending duodenum.

Image Interpretation

The stomach is severely repleted with echogenic fluid which is typical of a patient that has been persistently vomiting (image 2). Hyperperistalsis is evident along with the retention of ingesta (image 3). The luminal fluid dilation and hyperperistalsis continues into the descending duodenum (image 4). A translucent structure is obstructing the duodenal lumen consistent with a baby bottle nipple (image 5-7).

Outcome

Dale recovered uneventfully from surgery.

Clinical Differential Diagnosis

Upper gastrointestinal obstruction – foreign body/neoplasia, gastric ulceration, pancreatitis, or focal peritonitis.
