Diaphragmatic hernia in a 6 year old MN Pomeranian

The patient is a 6 year old neutered male Pomeranian dog, who was presented for occasional coughing. A right sided diaphragmatic hernia seen on radiographs. Blood work showed amylase 1583 and glucose 124; all other values were within normal limits.

 

DX

Right medial lobar hepatic diaphragmatic hernia.

Sonographic Differential Diagnosis

Right medial lobar hepatic diaphragmatic hernia, which appears stable. No regional inflammation was noted. However, surgical intervention should be considered given the coughing in this patient.

Image Interpretation

The herniated portion of the liver measured 8.2 cm and appeared to buttress against the vena cava. This appeared to be entering via diaphragmatic inlet and juxtaposed the right heart. The esophagus appears displaced cranially. Contiguous hepatic vein and portal vein surpass the diaphragm into the right medial lobes of the liver.
