A 13-year-old FS Dachshund with no obvious clinical signs was presented for evaluation of chronically elevated liver enzyme activity. Additional history was that in 2014 had had a left liver lobectomy, which on histopathology had been diagnosed with lymphoplasmacytic hepatitis with regenerative nodules. On physical examination, non-painful hepatomegaly and severe dental tartar was present. Bladder uroliths were evident on survey radiographs.

Sonographic Differential Diagnosis

Bilateral adrenal gland enlargement with remodeling.

Image Interpretation

The liver was coarse in architecture with increased portal markings. Uniform swelling of the caudate process was noted. This is most consistent with benign hepatopathy with inflammatory hepatopathy component. The gallbladder presented thin walls with normal, primarily anechoic content. The cystic and common bile ducts were normal. Residual cystic duct dilation was noted in this patient. This measured approximately 4.0 cm and created a new mucocele that is now deriving from the cystic duct. Suspended immobile debris was noted. The left adrenal gland was enlarged and irregular measuring 2.24 x 0.85 cm. The right adrenal gland was also mildly enlarged and heterogenous measuring 1.0 cm at the cranial pole and 0.8 cm at the caudal pole.

Clinical Differential Diagnosis

Liver – reactive hepatopathy, vacuolar hepatopathy, nodular regeneration, hepatoma, chronic low-grade inflammatory hepatitis, chronic-active hepatitis
Gall bladder – mucocele, cholecystitis
Urinary bladder uroliths
