CT – Subclinical chronic otitis media in a 10 year old FS DSH cat

This 10 year old FS DSH cat was recently diagnosed with intestinal lymphoma, being treated with lomustine and prednisolone. Presented with suspected retrobulbar mass. ALKP 166, ALT 409, Glu 381.

Image Interpretation

CT of the head, thorax and abdomen –

Head: Both tympanic bullae are completely filled with homogeneous soft tissue attenuating material without contrast enhancement.  The osseous wall of the tympanic bullae is bilaterally mildly thickened and slightly irregular and porous on the right side. Mild thickening and increased contrast enhancement of the luminal mucosal lining is noted bilaterally.

The retrobulbar spaces are unremarkable.

Thorax:  The ventral aspects of the right middle and the caudal compartment of the left cranial lung lobe show a very mild ground-glass opacity and mildly reduced volume.

 

DX

• Very mild compression atelectasis in the ventral aspect of both lung lobes • Moderate bilateral chronic otitis media: Right > left

Outcome

Cystic lesion papillary process of the liver; block vertebra L1/L2.
A retrobulbar mass was not identified and there is also no evidence of exophthalmus.

No structural changes associated with the previously diagnosed gastrointestinal lymphoma were noted.

The findings of both tympanic bullae are typical for subclinic chronic otitis media which is common in feline patients. A possible cause for this could be a subclinical rhinal disease like cat flu. However there is no macromorphologic evidence of relevant rhinitis at this point.
