CT – Soft tissue tongue neoplasia, lung fibrosis and possible adenocarcinoma recurrence post amputation in a 10 year old FS Maine Coon cat

This 10 year old FS Maine Coon cat has a history of apocrine carcinoma removed from toes. Left eye discharge, mass on tongue, skull base inflammation/mass seen on MRI, cranial nostril disease

DX

The findings at tongue are consistent with history of a soft tissue neoplasia – such as squamous cell carcinoma, melanoma, lymphosarcoma, other. The findings of cranial lung lobes are consistent with focal fibrosis of the lung parenchyma. Although it appears unlikely, there is a small chance for metastasizing neoplastic diseases by the known excised adenocarcinoma. The findings of both kidneys are consistent with chronic kidney disease, either chronic interstitial or glomerulonephritis. The soft tissue swellings localized to the amputation stumps are suspicious for reoccurrence of the adenocarcinoma even though granulation tissue formation secondary to surgery is a lower potential too.

Image Interpretation

CT of the head, thorax, pelvic limb – 

There is a mild amount of soft tissue attenuating material present in the left tympanic bulla.  The palatine bone presents marked atrophy with partial complete lysis of the bone plate.  The tongue presents a focal sublingual thickening at its mid body with irregular surface.

A focal irregular soft tissue attenuating area with small air-bronchograms is noted within the dorsal aspect of the right caudal lung lobe. Within the left and right cranial lung lobes two small sub-pleural soft tissue-attenuating foci are seen.  Both kidneys present small with moderate irregular margins. After contrast administration there is irregular triangular shaped filling defect of the renal cortex.  Multiple small round well-defined hypoattenuating areas are detected within the liver.

A moderately contrast enhancing, ill- defined, soft tissue swelling is seen at the distal aspect of the mid phalanx of the third and fourth toe of the left hind. The bony structures present no abnormal findings.
