This 5 year old MN Boxer presented with severe cervical pain, paresis of right front, ataxia all 4 limbs, low head carriage.

Image Interpretation

CT of the cervical and cranial thoracic spine – The computed tomography reveals a moderate to severe prolapse of heterogenously hyperattenuating disc materal into the ventral epidural space at C3/4. Symmetric distribution within the ventral epidural space is 

noted. The ventral subarachnoid space/contrast column is completely collapsed, the dorsal subarachnoid space is partially maintained but reduced in width. The extruded disc material occupies 50 % of the vertebral canal corss sectional area. The greatest extent is level with the intervertebral disc space. Cranial extension is limited to the caudal endplate of C3 but the caudal extension of the prolapse is up to the mid body of C4. The disc space C3/4 is moderately reduced in width. Multifocal partial mineralization of the discs is noted throughout the cervical and cranial thoracic spine and consistent with chondroid disc degeneration.

DX

The computed tomography reveals a moderate to severe disc extrusion after chondroid degeneration of the C3/4 intervertebral disc with moderate compressive myelopathy.

Outcome

The prolapsed material is in a symmetric position within the ventral epidural space.
Surgical decompression is indicated.
