CT of the head –
The caudal thirds of both nasal cavities are occupied by soft tissue attenuating material with multifocal contrast enhancement, nasal turbinate destruction and areas of anarchic mineralization. There is multifocal lysis oft he nasal septum as well as a focal leftward deviation of the nasal septum associated with the lesion in the right nasal cavity. The nasal bones do not present osteolysis. Both frontal sinuses are occupied with fluid secretions and present swelling of the mucosal lining. Extensive nasal turbinate destruction is noted within therostral thirds of both nasal cavities.
Even though a neoplasia appears to be most likely fungal granulomas have to be considered a potential differential diagnosis owing to the multifocal/bilateral character and aggressive behavior.
Round cell neoplasia – such as lymphoma – appears more likely than adenocarcinoma or other because oft he bilateral distribution and relatively mild bone destruction.
Tissue sampling for histopathology and fungal culture is required for further definition here.
