This 14 year old FS mixed breed dog presented with a cough, suspect thoracic mass.

DX

The findings confirm the suspected diagnosis of pulmonary neoplasia which is consistent with history of chronic cough due to bronchial compression.

Image Interpretation

CT of the thorax – 

 

 An ill-defined uniformly soft tissue attenuating ovoid and heterogenenously contrast enhancing bronchocentric mass lesion is seen at the level of the division of second to third degree bronchi within the right caudal lung lobe. The mass measurements are 3.1 x 3.0 x 3.3 cm. The lateral branching bronchus is completely obstructed by the mass lesion and the associated lung segment presents a reduced volume and increased patchy soft tissue attenuation with small airbronchograms. The remainder of the distal bronchi are partially compressed. 

 

 A hypoattenuating and hypoenhancing liver well-defined ovoid lesion of 1.3 cm diameter is seen within the craniodorsal aspect of the right medial liver lobe at the very edge of the scan. 

Outcome

The mass lesion is highly suspicious for bronchoalveolar carcinoma. Squamous cell carcinoma, pulmonary or histiocytic sarcoma would be possible differentials.
Theoretically further definition by ultrasound guided FNA sampling of the mass lesion might be considered. (The lesion should be accessible via the dorsal third of the right 9th intercostal space after right lateral recumbent positioning to provoke atelectasis of the aerated parenchyma superficial to the mass.)
However, the therapy of choice is lobectomy of the right caudal lung lobe more or less regardless of the FNA results.
