CT – Primary idiopathic epilepsy likely in a 2 year old MN dog with seizures and normal head

This 2 year old MN dog has a history of seizures and has been on Phenobarbital for 1 month. Presented for 6 cluster seizures.

DX

The computed tomography of the head is within normal limits. There is no structural evidence of rhinitis at this point.

Image Interpretation

CT of the head, plain and post contrast – The computed tomography of the head is within normal limits.
Subtle incidental asymmetry of the lateral ventricels is noted. Except for this the anatomy, attenuation and enhancement of the brain parenchyma is as expected.

Outcome

With regards to the age of the patient primary idiopathic epilepsy is the most likely underlying cause of the seizural activity here.

Cranial fossa diseases that are associated with macroscopic structural changes can be ruled out largely. However, as inflammatory, metabolic/toxic and neurodegenerative disorders cannot be ruled out based on negative imaging findings the study should be complemented by a CSF examination at least. In case of the strong clinical suspicion of caudal fossa/brain stem involvement or should the imaing finidngs not parallel the clinical context MRI would be another option for further definition.
