Image Interpretation

CT of the skull and thorax: 

skull- 

There is moderate to marked horizontal bone loss in all jaw quadrants. Level with the remaining alveolar bone of the absent tooth element 104 marked atrophy with focal loss of the alveolar bone demarcating the right nasal cavity is noted. Level with the absent tooth element 210 the alveolar bone presents focal moth-eaten osteolysis without adjacent soft tissue swelling or contrast enhancement.

Thorax – wnl

DX

The findings are consistent with a focal osteomyelitis level with the absent triadan 210.

Outcome

A primary neoplastic origin (osteosarcoma, odontogenic tumor) is very unlikely. The epistaxis cannot be explained by the focal bone loss level with triadan 104 as the
bony opening is still covered by soft tissue at this point and neither a focal soft tissue
reaction nor nasal secretions are seen.
Based on the findings oral bleeding due to chronic gingivitis appears most likely.
