CT – Oronasal fistula secondary to tooth fracture with rhinitis in a 12 year old FS West Highland Terrier

This 12 year old FS West Highland Terrier presented with acute onset reverse sneezing, anorexia, light pink nasal discharge rght nares. Dog has no significant evidence of dental disease.

Image Interpretation

CT of the head- A moderate amount of gravity-dependent soft tissue attenuating material is seen along the nasal turbinates within the rostral aspect of the right nasal cavity. After contrast administration the mucosal lining of the right nasal turbinates presents moderate thickening. The palatinal mesial tooth root of triadan 108 presents a transverse fracture line at its neck. The periodontal ligament and dural lamina of this tooth root are lost. The apex  of the tooth root extends into the nasal cavity. The tooth root apex is capped by a soft tissue attenuating nodule of 1 x 0.8 x 1 cm with moderate heterogeneous contrast enhancement.

The palatinal mesial root of triadan 208 presents a transverse fracture. The distal roots of the triadan 108/208 and the buccal mesial tooth root of 209 are surrounded by halos of moth eaten ostelysis with complete focal destruction of the lateral maxillary cortex. 

There are multiple complicated fractured tooth roots affecting triadan 201, 206, 303, 402 and 403.  There is a moderate generalized horizontal loss of the alveolar bone in all four quadrants.

DX

• Chronic tooth root fracture triadan 108 with oro-nasal fistula, apical tooth root granuloma and secondary chronic rhinitis with focal destrusion of the nasal turbinates

Outcome

The findings are consistent with with an oronasal fistula secondary to a fracture tooth root of triadan 108 with a focal granuloma in the right ventral meatus nasi. The rhinitis is secondary to the dental disease.
