DX

Nasopharyngeal soft tissue neoplasia meeting malignancy criteria causing nasopharyngeal stenosis and bilateral secretory otitis • Regional metastatic spread to the right medial retropharyngeal lymph node • Chronic inflammatory nasal polyp in nasal fundus. Theoretically multifocal manifestation oft he primary tumor is a potential too.

Image Interpretation

CT of the head and thorax – The computed tomography of the head reveals a soft tissue mass emerging from the nasopharyngeal roof and obliterating the nasopharyngeal airways.  completely. The mass measures 2.5 x 2.5 x 1.5 cm. The mass effect causes ventral displacement of the soft palate and bilaterally symmetric secretory otitis media. Non-uniform contrast enhancement sparing the central region is noted within the mass. The right medial retropharyngeal lymph node presents severe asymmetric enlargement with markedly heterogenous contrast enhancement sparing the central regions. The short-to-long-axis ratio of the lymph node has increased to > 0.5. A small well-delineated round polypoid soft tissue mass of 5 mm diameter emerging from the ventral nasal concha is seen on the left side of the nasal fundus.

Outcome

Normal geriatric thorax. Consider round cell neoplasia, such as lymphoma, squamous cell. carcinoma, melanoma, fibrosarcoma and other as differentials. Sampling of the primary tumor is the most reasonable option here to set up a treatment plan since regional metastatic spread is evident.
The prognosis is guarded.
