Case Study

This 15 year old FS DSH cat has a history of right sided sarcoma from 2011. Presented with nasal/ocular discharge and weight loss.

Image Interpretation

CT of the head-

The right nasal cavity is occupied by a contrast enhancing soft tissue mass which causes multifocal aggressive osteolysis of the nasal turbinates, nasal, vomer, maxillary, and palatinal bone as well as lysis of the bony orbita and nasal septum. The mass penetrates into the right retrobulbar space and left nasal cavity as well as towards the dorsum of the nose. The cribriform plate is intact. The dependent portion of the right frontal sinus contains a moderate amount of fluid with a horizontal fluid level. The right submandibular lymph nodes present moderate symmetric enlargement with a regular contrast enhancement pattern. The medial retropharyngeal lymph nodes are within normal limits.

DX

• malignant nasal neoplasia with aggressive bone destruction • regional lymphadenomegaly with a reactive pattern • secretory sinusitis

Outcome

The computed tomographic findings are compatible with recurrence of the previous fibrosarcoma. Curative surgical resection is not an option as to the extension of the tumor. Cytoreductive surgery, chemotherapy and/or radiative treatment may be considered as palliative treatment options. The overall prognosis is poor.
