DX

The current computed tomographic study is consistent with multifocal reoccurrence of the hemangiosarcoma along the thoracic wall and the soft tissues lateral to the lumbar epaxial musculature. Moreover a possible focus of intraperitoneal seeding is noted.

Image Interpretation

CT of the skull and thorax – 

Multiple pulmonary osteomas are noted. There is no evidence of metastatic spread to the lung parenchyma, mediastinal lymph nodes or heart base.  3 new irregular contrast enhancing ovoid subcutaneous soft tissue attenuating, nonuniformly enhancing mass lesions are seen lateral to the left 6th to 8th rib, the largest one measures 1.4 x 1.7 x 3.7 cm.  well defined soft tissue attenuating nodules are noted ateral to the epaxial muscles, measuring between 1.6 to 3.8 cm. Compared with the previous study their size has increased remarkably. 

The two nodules within the splenic body present with similar size and enhancement pattern compared with the previous  study. There is no evidence of macroscopic metastatic spread to the liver or abdominal lymph nodes. However, a new intraperitoneal soft tissue nodule of 2.2 cm diameter is seen on the ventral abdominal wall in a midline position.

 

Outcome

The long-term prognosis for the patient is poor due to multifocal metastatic spread
