CT – Metastatic lung disease noted in a 17 year old FS West Highland Terrier presented with neurological signs

This 17 year old FS West Highland Terrier has a history of CRF (iris stage 4). Presented two weeks ago with rear limb ataxia; focal seizures began two days ago and presented to the hospital non- responsive. PE: dull, minimally responsive, sever neck pain on lateral and vertical movement. CBC/Chem: Crea 6.5, BUN elevated

DX

An underlying cause of the neurological signs was not seen. However, there were changes suggestive for metastatic lung disease.

Image Interpretation

CT of the head and neck – 

The right ocular bulb is small and ovoid in shape with deviation from normal intraocular anatomy. The area of the vitreal chamber presents increased attenuation. The right maxilla presents marked bone atrophy of the alveolar bone.  Abnormal enlargement and asymmetry of the lateral ventricles of the brain is noted.  The pictured parts of the cranial lung lobes present a multifocal mixed nodular pattern and hydrostatic atelectasis.

Outcome

Confirmation of the findings either by means of radiography or computed
tomography should be considered. Consider an abdominal ultrasound to identify a
possible primary tumor as well. If further definition of the neurologic disease is
warranted MRI may be considered. The macromorpholical findings of the right ocular bulb are consistent with chronic
intraocular disease, for example chronic uveitis and are not related to the present
clinical signs.
