CT- medal coronoid pathology (MCP) and elbow joint incongruity with kissing lesion in a 3 year old FS Labrador Retriever dog

 This 3 year old FS Labrador Retriever dog presented with a history of right front lameness of 6 months duration. Previous rads and CT of shoulder showed no cause for lameness.

DX

The CT reveals medial coronoid pathology (MCP) with fissures and complex elbow joint incongruity with a long ulna. The findings are further suggestive for a kissing lesion within the humeral trochlea.

Image Interpretation

CT of the right elbow, plain and post contrast – Multiple fissure lines are seen at the tip of the medial coronoid process. The longest fissure line parallels the radioulnar space and partially isolates a flake-shaped fragment. The outline of the medial coronoid process is irregular. The tip of the medial coronoid process is heterogenous in opacity whereas the base reveals marked sclerosis. The radioulnar space is incongruent and narrow. There is a mild radioulnar step formation of 1 mm with a long ulna as well as mild asymmetry of the humeroulnar joint space.
Focal sclerosis of the subchondral bone of the humeral trochlea is noted.
A mild amount of osteophytes is noted at the periarticular margins.

Outcome

The asymmetric longitudinal growth between the radius and ulna is likely to be the underlying cause of the medial coronoid disease.
Mild elbow joint osteoarthritis is noted as a consequence of the MCP. Progression of the degenerative joint disease has to be expected.
Arthroscopic (or arthrotomic) revision of the joint is recommended.
