Image Interpretation

CT of the neck and thorax – 

Neck:

A ovoid 5 x 4 x 5 cm large soft tissue mass with chaotic mineralization and non-uniform contrast enhancement is emerging form the base of the tongue. The leion margns are ill-defined. The basihyoid bone is included in the mass and presents severe aggressive osteolysis. There is a significant mass effect on the larynx emphasizing the base of the tongue and epiglottis and causing laryngopharyngeal stenosis. Multiple large meandering vascular anastomoses are emerging from the base of the tongue.

Thorax:

The pulmonary parenchyma and mediastinal lymph nodes are negative for metastatic spread

DX

Outcome

Ectopic thyroid carcinoma (embryonal development involves the region of the base of the tongue), laryngeal carcinoma, osteosarcoma and chondrosarcoma of the hyoid bone are likely differential diagnoses amongst others.

Final diagnosis warrants tumor sampling. The prognosis for curative surgical resection is poor owing to the infiltrative growth into critical structures. However, at least cytoreductive surgery followed by radiative treatment and/or adjuvant chemotherpy may be an option depending on the tumor type.

The computed tomography is negative for regional and distant metastatic spread.
