DX

The computed tomographic findings are compatible with a malignant nasal soft tissue neoplasia with secondary aggressive osteolysis, invasion of the cranial vault and upper respiratory tract obstruction.

Image Interpretation

CT of the head and thorax – 

A large soft tissue attenuating mass is seen obiliterating the entire left and part of the right nasal cavity.  Aggressive permeative osteolysis of the maxillary, palatinal, frontal and left orbital bone is seen as well as osteolysis of the nasal septum, cribriform plate and nasal turbinates. The mass shows expansile growth and perforation into the right nasal and into the oral cavity as well as into the cranial fossa left orbita and onto the dorsum of the nose. A mild mass effect onto the left olfactory bulb is noted. Increase of nasal and frontal sinus secretions is noted. The nasal fundus is completely, the nasopharynx partially obstructed. There is no evidence of metastatic spread to the interstitium of the lung or mediastinal lymph nodes. 

Outcome

Likely differentials include adenocarcinoma, squamous cell carcinoma, lymphosarcoma, melanoma, fibrosarcoma and esthesioneuroblastoma. Negative for metastatic spread.
There is secondary secretory frontal sinusitis and rhinitis/hemorrhage. Full tumor staging would require sampling of the lymph nodes and abdominal ultrasound next to primary tumor biopsy.
