This 2 year old M Pit Bull Terrier presented for a right thoracic wall mass that has enlarged over the previous 6 weeks. Inappetance and weight loss. Staging CT.

DX

Malignant mesenchymal neoplasia of the 5th right rib: most likely chondorsarcoma, osteosarcoma, fibro- or hemangiosarcoma

Image Interpretation

CT of the thorax and abdomen – 

A large ovoid non-uniformly enhancing extrapleural soft tissue mass measuring 10 x 15 x 10 cm is centered around the right 5th rib and causes severe aggressive permeative osteolysis of the mid and ventral third of the rib. The 4th and 6th rib present extensive amorphous periosteal reactions. There is a severe mass effect on the right pleural cavity causing compression and displacement of the right lung as well as leftward shift of the heart with mild compression of the right ventricle.  A soft tissue opaque interstitial noduole of 8 mm diameter is see at the border between the right middle and caudal lobe.  The sternal and cranial mediastinal lymph nodes present moderate asymmetric enlargement and rounding with an increased short-to-long-axis ratio > 0.5 as well as heterogeneous contrast enhancement.   The splenic tail presents thickening with nodular protrusions and heterogenous contrast enhancement.

Outcome

• Extensive peripheral soft tissue edema: neoplastic versus reactive inflammatory infiltrate or less likely hematoma
• Lung nodule, sternal and cranial mediastinal lymphadenomegaly meeting metastatic criteria
Splenic nodules: benign hyperplasia vs. Metastatic spread need to be considered.
