CT – Intervertebral disc extrusion at T12/T13 with compressive myelopathy in a 3 year old F Dachshund

This 3 year old intact F Dachshund presented with sudden onset of paraparesis. Normal to hyper reflexia hind limbs, no CP deficits.

DX

The findings are consistent with acute right sided intervertebral disc extrusion of mineralized material at the level T12/T13 with marked compressive myelopathy

Image Interpretation

CT of the cervical, thoracic and lumbar spine – 

At the level of the intervertebral disc space T12/T13 irregular mineral-attenuating disc material is present in the right ventral epidural space. It occupies approximately 70 % of the cross sectional area of the vertebral canal at this level. The material extends cranially up to level of the cranial vertebral endplate of T12 and caudally to the mid aspect of the vertebral body of T13. After intrathecal contrast administration Right lateroventral compression of the subarachnoideal space at the level T12/T13 is noted. A mild amount of hyperattenuating material extends caudally to the level of L2, consistent with hemorrhage.  A small amount of mineral-attenuating material is noted within the entrance zone of left neuroforamen L5/L6.

Associated with all intervertebral discs ovoid mineral-attenuating areas are noted. Mild spondylosis formation L2/L3.

Outcome

Acute intervertebral disc extrusion and compressive myelopathy at T12/T13
with extensive non-compressive hemorrhage migrating caudally within the
epidural space
• Mild lateralized intervertebral disc extrusion L5/L6 with moderate left sided
neuroforaminal stenosis – likely incidental
• Multifocal chondroid disc degeneration of all intervertebral discs of the
cervical, thoracic and lumbar spine
Surgical decompression is advised.
