DX

The computed tomographic findings are compatible with a malignant right-sided soft tissue neoplasia with secondary aggressive osteolysis and intracranial expansion.

Image Interpretation

CT of the head – 

The computed tomography of the head reveals a large soft tissue attenuating and non-uniformly enhancing expansile mass lesion of 8 x 4 x 4 cm on the right side of the skull with infiltrative growth into the right orbita, right frontal sinus and cranial fossa level with the temporal bone. Multifocal polyostotis permeative osteolysis of the calvarium, frontal sinus, temporomandibular joint and coronoid process of the mandible is noted. Meningeal thickening with increased contrast enhancement is evident in the right temporal lobe region.  A moderate exophthalmus with dorsal and temporal deviation of the bulbus is noted. The rightsided medial retropharyngeal and submandibular lymph nodes present moderate enlargement with heterogenous contrast enhancement. The right ear canal is deviated caudally.

Outcome

Possible differentials include squamous cell carcinoma, fibrosarcoma, hemangiosarcoma, lymphosarcoma, melanoma, myxosarcoma and other.
The computed tomography furtehr is suggestive for metastatic spread to the regional lymph nodes.
