Case Study

CT – Fissuring of the Medial Coronoid Process and Emerging Osteoarthritis in a 6 month old M Rottweiller dog

History of noted recent gait change. Physical exam showed mild right front limb lameness, mild elbow joint discomfort

History of noted recent gait change. Physical exam showed mild right front limb lameness, mild elbow joint discomfort

DX

The computed tomographic findings are compatible with medial coronoid pathology with fissuring of the medial coronoid process and emerging osteoarthritis.

Image Interpretation

CT of the elbows – The tip of the medial coronoid process was irregular in outline with a small hook-shaped extension. There was a faint fissure line separating the tip of the medial coronoid process incompletely. The base of the coronoid process was increased in opacity and presented loss of trabecular pattern. The abaxial surface of the medial coronoid process was blurred and presented a mild and heterogenous decrease in opacity.
The subchondral bone and periarticular bone presented few small cystic lesions.
There was no subchondral bone defect within the humeral trochlea. The joint surfaces appeared to be congruent.

Outcome

The CT findings are only mild and their relevance is variable – especially in Rottweilers. In case of matching clinical signs arthroscopic revision of the joint is recommended. Prior to that the contralateral limb should be assessed for medial coronoid pathology/elbow dysplasia too since this rarely occurs as a unilateral disease.
