History of weakness in the hind legs and reluctance to stand for more thatn a minute or two. Physical exam: diagnosed with hip dysplasia at the age of nine months. Presented for hind limb weakness.

DX

T13-L1 ventral right-sided mineralized disc extrusion with moderate cord compression; L3-4 ventral right-sided mineralized disc extrusion with mild cord compression; abdominal adenopathy (reactive v. metastatic); limbo-sacral disease, possibly dynamic cauda equina

Image Interpretation

CT of the spine- Thoracic Spine: At T13-L1 there is a ventral, right-sided spinal canal mass effect, causing leftwards deviation and moderate compression of the spinal cord.
Lumbar spine: At L1-2 there is a ventral, spinal canal mass effect, causing mild dorsal deviation but no compression of the spinal cord. At L3-4 there is a ventral, right-sided spinal canal mass effect, causing leftwards deviation and mild compression of the spinal cord
Sacral spine: There is a step defect at LS junction; the sacrum is 3-4 mm ventrally displaced relative to the caudal end plate of L7m.
Abdomen: left sided enlarged structure (splenic LN?): nodule; poorly enhancing centrally (2.1 cm wide). Additional jejunal and, possibly, colic nodes also mildly enlarged

Outcome

Recommendations are surgical decompression of T13-L1; Consider surgery at L3-4; Consider abdominal ultrasound; Dynamic imaging for possible lumbo-sacral disease
