This 11 month old Himalayan cat has a history of pyrexia with nasal discharge; fever has resolved but nasal discharge is persistent. Fungal vs other; biopsy pending.

DX

• Destructive leftsided rhinitis with sinusitis • Moderate to severe lymphadenitis of all tributary lymph nodes of the head

Image Interpretation

CT of the head – The left frontal sinus is partially filled with soft tissue attenuating, non-enhancing material. Multifocal accumulations of non-gravity dependent soft tissue attenuating material are asymmetrically distributed between the conchae of the left nasal cavity. The material presents no contrast enhancement. There is moderate turbinate destruction destruction. The submandibular, parotid and medial retropharyngeal lymph nodes present irregular enlargement and contrast enhancement.

Outcome

The most likely underlying disease is fungal rhinitis such as cryptococcosis with metastatic lymphadenitis.
Bacterial/mycobacterial rhinitis and the simultaneous presence of chronic destructive rhinitis with a neoplasia of the lymphatic system cannot be ruled out entirely but is considered low for potential.
Rhinoscopic sampling for bacterial and fungal culture as well as for histopathology (the diagnosis of fungal rhinitis is more straightforward microscopically at times) is advised if not yet done so.
Moreover fine needle aspiration, if not biopsy or nodulectomy (left medial retropharyngeal lymph node would be ideal here) of the lymph node is strongly recommended for further definition.
