CT – Degenerative joint disease left and right elbow joints in a 1 year old MN Pomeranian

This 1 year old MN Pomeranian presented for evaluation of grade II lameness right thoracic limb of several weeks duration. Painful on extension. Rads show OA of right elbow and bone lysis/remodeling of the anconeus.

Image Interpretation

CT of the front limbs – 

left: 

The left elbow joint presents a mild radioulnar step formation with a long ulna. The periarticular margins of the elbow joint present mild osteophyte formations.

right: 

The periarticular margins of the elbow joint present mild osteophyte formations. The medial humeral condyle presents moderate regional sclerosis of the subchondral bone. A moderate radioulnar step formation with long ulna is noted as well as crescent shaped asymmetry of the humeroulnar joint space. Moderate sclerosis is noted at the caudal aspect of the ulnar notch.

The anconeal process is completely fused.

DX

The findings are consistent with emerging degenerative joint disease of both elbow joints due to incongruity.

Outcome

On the right side there is evidence of subchondral bone
sclerosis due to increased pressure on the medial humeral condyle. Emerging cartilage
breakdown is assumed in the medial compartment of the right elbow joint. In the current study there is no evidence for coronoid disease or isolated anconeal
process. Ulna-osteotomy is advised to reduce the possible compartment syndrome of the medial
aspect of the right elbow joint. Arthroscopy should be performed to evaluate joint
cartilage and check medial coronoid process for possible cartilaginous fragmentation
that is not noticeable via CT.
