DX

The computed tomographic findings are compatible with atlatoaxial subluxation syndrome with hypoplasia of the Dens axis.

Image Interpretation

CT of the cervical spine – 

The computed tomography reveals upper cervical stenosis due to atlantoaxial subluxation. The Dens of the axis is hypoplastic. There is widening of the space between the vertebral arch of the atlas and the spinous process of the axis. Due to the dorsal displacement of the Dens the vertebral canal diameter is reduced by almost 50% with upper cervical kyphosis & medullary kinking.  The shape of the caudal cranial fossa appears to be asymmetric.

Outcome

Traumatic injury of the upper cervical spinal cord is possible.
Correlate the findings with the MRI to assess for parenchymal damage and
abnormalities of the ligamentous structures in the occipitoatlantoaxial region.
Surgical stabilization techniques are likely required to prevent further damage.
