Chronic renal disease with infarction in a 7 year old MN DSH cat

A 7 year old neutered male DSH cat was presented with a history of azotemia, polyuria, polydypsia, and weight loss. Physical examination and blood pressure were both within normal limits. The only significant abnormality on urinalysis was a subnormal specific gravity. Abnormalities on blood chemistry included azotemia and hypercalcemia.

DX

Chronic renal disease with infarction and pyelectasia

Sonographic Differential Diagnosis

Chronic renal disease, interstitial degenerative pattern with cortical infarction. Potential concurrent infection suggested by the mild pyelectasia. (Pyelectasia may occur also in case of diuresis or scarring).

Image Interpretation

The kidney in this patient shows diffuse increased cortical echogenicity with a focal hyperechoic infarction at the caudal pole and collapse of the respective corticomedullary junction. Slight pyelectasia is evident.

Outcome

The patient was treated for emerging renal failure with diet, antibiotics, and fluids. He was eventually was lost to follow-up.

Primary chronic kidney disease (CKD), renal disease secondary to hypercalcemia (neoplasia, idiopathic, granulomatous disease, vitamin D toxicity).
