An 18-year-old FS DSH was presented for evaluation of weight loss and vomiting, Eosinophilia was present on CBC. The only abnormality on urinalysis was glycosuria.

An 18-year-old FS DSH was presented for evaluation of weight loss and vomiting, Eosinophilia was present on CBC. The only abnormality on urinalysis was glycosuria.

DX

Chronic active pancreatitis, potential carcinoma.

Sonographic Differential Diagnosis

Chronic active pancreatitis, potential carcinoma. FNA of the pancreas would be recommended. Guarded prognosis.

Image Interpretation

The pancreas revealed mixed, hypoechoic changes with swollen contour in the left limb. FNA would be warranted. The pancreatic duct was visualized and measures within normal limits at 0.17cm. There is minor reactive appearing, coarse fat around the capsule. This is consistent with chronic active disease. Pancreatitis is likely; however, carcinoma cannot be entirely ruled out. FNA of the pancreas would be recommended as it appears to be adjacent to the body wall and clear sampling avenue should be feasible. This region measured 3.2 x 1.1 cm.

Outcome

Per the owner, the patient was doing well and on steroid therapy.

Metabolic – diabetes mellitus, hyperthyroidism. GI tract – neoplasia, foreign body, IBD, parasitic.
Pancreas – pancreatitis, neoplasia.
