Bladder mass, suspect transitional cell carcinoma, in a 14 year old FS DSH cat

A 14-year-old FS DSH was presented for evaluation of chronic cystitis with current signs indicative of FLUTD. Therapy with Baytril, Buprenex, and Prazosin had shown no response. CBC, serum biochemistry, and T4 were all within reference range. Urinalysis showed SG 1.052, 1+ protein, and 3+ struvite crystals. On survey radiographs, the kidneys had a mottled appearance and there was subtle radio-dense “specks” within the urinary bladder.

DX

Image Interpretation

The urinary bladder presented a 1.0 cm, echogenic clot. The clot was mobile within the bladder. The bladder revealed a 1.45 cm dorsal mass with focal mineralization. The mass was approximately 0.5 cm cranial to the right ureteral papilla.  Color Doppler assessment revealed vascularity of the mass. The mass appears resectable and was attached by a pedicle at the dorsal wall. Some suspended debris was noted. The cystourethral junction and urethra appeared free of evident pathology.

The stomach was repleted with progressively shadowing material. This is most consistent with hair accumulation especially if the patient was n.p.o. at the time of the sonogram.

 

Outcome

Unremarkable kidneys, age-related pancreatic changes. Surgical intervention with resection of the dorsal bladder wall is recommended.

Resistant bacterial cystitis, interstitial cystitis, uroliths, neoplasia, polyploid cystitis
