Bladder mass in a 12 year old FS German Shepherd mix

A 12 year-old spayed female Shepherd cross dog was present for hematuria with blood clots with a history of having Addison’s disease and uveitis. Urine was turbid in appearance and contained 3+ protein, 3+ blood, and elevated microalbuminuria. Abnormalities on CBC and serum biochemistry included monocytosis, hypoalbuminemia, elevated AST and ALT activity, and hypocalcemia. Coagulation panel was normal.

Sonographic Differential Diagnosis

Either attached blood clot or minor possibility of low grade neoplasia. Rule out coagulopathy.

Image Interpretation

The urinary bladder contained a 5.4 x 2.6 cm attached blood clot to the dorsal wall or potential low grade neoplasia.

Clinical Differential Diagnosis

Hematuria – bladder pathology (bacterial cystitis, neoplasia, uroliths, trauma, sterile cystitis), renal pathology (pyelonephritis, neoplasia, renoliths, trauma, glomerulonephritis). Liver disease.

Sampling

Aspirate biopsy of the bladder mass revealed mostly frank blood with some low grade, mixed inflammation.
