Sonographic Differential Diagnosis
Degenerative Mitral Valve Disease (DMVD), likely accompanied by mitral valve stenosis. Moderate mitral insufficiency, and moderate left atrial enlargement.
Image Interpretation
There is a mildly volume overloaded left ventricle, a moderately enlarged left atrium, and a markedly thickened (degenerated) mitral valve. Moreover the mitral valve leaflets do not appear to open properly, which could be due to congenital mitral valve stenosis. There is moderate mitral insufficiency present.
DX
DMVD, mitral valve stenosis. Moderate mitral insufficiency, moderate left atrial enlargement.
Outcome
If the dog does not show any clinical signs at the moment, no therapy would be recommended, although the dog should be rechecked in 3 months. If there are clinical signs, triple therapy (pimobendan, ACEI, and furosemide) should be initiated. This patient should not be bred.
Comments
A low furosemide dosage would be recommended due to suspected mitral stenosis, as a higher furosemide dosage may lead to decreased left ventricular filling.
