Mild HCM

Sonographic Differential Diagnosis

Based on the mildly hypertrophied basal interventricular septum this seems to be a mild form of Hypertrophic Cardiomyopathy. Although SAM could also be caused by some form of mitral valve dysplasia with lengthening of the anterior mitral leaflet I would still think itВґs a sequelae of HCM in this case because the heart murmur was first heard at the age of seven and there is a small area of hypertrophy present. Based on findings (mild HCM, no atrial enlargement) a recheck is recommended yearly. ItВґs likely that the patient will stay free of symptoms for a long period of time.

Image Interpretation

Normal left ventricular and atrial dimensions with a slight thickening of the basal septal area was noted. The right ventricle and atrium are normal too. On the right parasternal five chamber view systolic anterior motion of the mitral valve is clearly visible.

Clinical Differential Diagnosis

Mild cardiomyopathy (hypertrophic/dilated), degenerative valve disease, incidental congenital defect (ASD, VSD, tricuspid dysplasia)
