An 8-year-old intact female King Charles that had an enlarged left LA on previous echocardiography was presented for ???. Current therapy is Lasix, Benazepril, and pimobendan. Systolic BP is within normal range.

Sonographic Differential Diagnosis

Severe DMVD with CHF and systolic dysfunction, possibly aggravated by chordal rupture The therapy you started is perfect (I assume the dog has 5 kg?). Perhaps you can add Spironolactone too. I would adjust the Furosemide dosage to the findings on radiographs (look at what the lung pattern says). Prognosis: Usually the prognosis for a dog with CHF due to DMVD is not longer than one year (but cannot be defined for the individual animal)

Image Interpretation

What I can see is severe left ventricular volume overload with septal hyperkenesis, a markedly enlarged left atrium, moderately thickened mitral and tricuspid valve leaflets, marked mitral valve prolapsed with a flail posterior leaflet possibly due to chordae rupture (can be seen in one oblique right parasternal clip). There is severe mitral insufficiency and mild tricuspid insufficiency present. Compared to the echo done in Sept. there is obvious deterioration visible (due to chordae rupture). Flow velocities across the LVOT/RVOT are normal, but the flow profile across the RVOT seems to be very asymmetrical (shorter acceleration time) indicating pulmonary hypertension due to CHF.
