An 8-year-old, intact male Mastiff was presented for hematuria. Blood work showed WBC 18,000 with neutrophilia, globulins 5.3, urine SP 1.052 with 3+ protein, and >50 WBCs. Radiographs revealed prostatomegaly.

Sonographic Differential Diagnosis

Prostatic abscesses.

Image Interpretation

The urinary bladder presented a moderate amount of suspended and dependent debris.
The prostate in this patient was severely enlarged and measured 12.1 x 5.5 cm with heterogenous abscessing cysts that measured 4 cm, 3 cm, and 2 cm. Chronic hyperechoic remodeling of the prostate and pericapsular inflammatory pattern was noted. Focal areas of mineralization were noted. This is consistent with dystrophic mineralization with a minor potential for neoplasia. No parenchymal masses were noted. The testicles were imaged and found to be echogenic with mild areas of remodeling.
