A 10-year-old NM Rhodesian Ridgeback was presented for evaluation of tenesmus and abnormal urination. CBC showed leukocytosis with neutrophilia and monocytosis. On urinalysis, specific gravity was normal (1.041) but hematuria and pyuria were present.

Sonographic Differential Diagnosis

Prostatic mass with cysts/abscesses. Echogenic debris.
Bilateral, ureteral dilation and mild, bilateral pyelectasia/emerging hydronephrosis.
Ultrasound-guided drainage of the prostatic cyst would be recommended. FNAs were performed without difficulty.

Image Interpretation

The prostate in this patient was enlarged with mixed, echogenic changes with a 1.5 cm echogenic cyst with potential abscessation. Capsular expansion and pericapsular inflammatory pattern was noted throughout the fat in the pelvic inlet. Mineralization of the prostate was noted and strongly suggestive for prostatic carcinoma.
The urinary bladder, trigone and pelvic urethra presented normal wall thicknesses with anechoic urine and normal tone. Echogenic debris was noted throughout the urinary bladder. No evidence of inflammatory or neoplastic changes were noted.
The ureters were dilated and obstructed owing to impingement by the prostate that is enveloping the cystourethral junction. The left kidney presented mild pyelectasia that measured 1.01 cm. The left kidney measured 9.05 cm. Left ureteral dilation measured 0.77 cm with pyelectasia that measured 2 x 1 cm. Right kidney pyelectasia was also noted and measured 0.5 x 1.5 cm. The right kidney measured 8.6 cm. The right ureter was dilated at 0.77 cm.
