06_00327 Riley M Right renal abscess

A 1-year-old SF Labrador with a long history of a mass effect of the right kidney and right flank abscess with two prior surgical debridements, was presented for evaluation. CBC showed severe leukocytosis (51,000). On urinalysis, proteinuria, hematuria, and leukouria were present.

 

DX

Right renal abscess

Sonographic Differential Diagnosis

Abscess in region of the right kidney with free fluid and peritonitis. Surgical consultation is recommended with immediate exploratory. Removal of the abscess/residual right kidney is likely necessary in this patient with abdominal lavage.

Image Interpretation

The left kidney was uniform in size and contour measuring 9.15 cm. The right retroperitoneal region revealed a 6.0 cm abscess with mixed hyperechoic and hypoechoic changes and regional inflammatory pattern. The right kidney was not visualized in this patient. However, the abscess was in the region of the right kidney. The abscess is likely residual structure from a prior right kidney. Surgical consultation is recommended. The center of the structure reveals a 3.0 cm echogenic tissue structure. This is consistent with residual right kidney.
Free fluid was noted in the abdomen with reactive fat.
