Sonographic Differential Diagnosis

Ectopic left ureter, suspect intramural. Ectopy entering into the pelvic urethra through the trigone. Secondary moderate hydronephrosis of the left kidney. The left kidney may need to be removed and would be a judgment call at the time of surgery. Recommend surgical referral for this patient.

Image Interpretation

The urinary bladder in this patient presented significant distension and suspended debris. A 1.5 cm dilated ectopic ureter was noted and appeared to be entering into the urinary bladder and then enter into the pelvic urethra, which makes it an intramural ectopic ureter. It was tortuous with echogenic urine. This extended cranially to the left kidney, which was significantly dysplastic with moderate hydronephrosis. The right kidney was structurally unremarkable with slight pyelectasia. The right kidney was normal in size at 5.0 cm.

DX

Ectopic ureter

Outcome

Surgery was performed and the patient had an excretory urogram. The right kidney was fairly normal with a normal right ureter. Left ureter was attached to bladder neck. In surgery the left kidney and ureter were removed, the ureter was attached to the bladder neck/proximal urethra in the pelvic canal. Extramural.
